If you have worked for different employers, you need these forms from each of them.
If you do not have the P45 and/or P60 form, you can submit the most recent payslips.
Copy of registration in the Construction Industry System (CIS) with an individual taxpayer number, if you have one.
If you were self-employed (with UTR) we will need one of the following: Monthly contractor statements, payment statements (CIS) or subcontractor bank statements.
